Data Breach AlertJuly 15, 2022 – Family Practice Center PC reported a data breach after the company was targeted in a recent cybersecurity attack. As a result of the breach, the name, Social Security number, address, medical insurance information, health and treatment information of certain individuals was compromised. The Family Practice Center data breach is believed to have impacted as many as 83,969 individuals. On July 7, 2022, Family Practice Center PC sent out data breach letters to those individuals whose information was affected by the breach.

Family Practice Center PC (“Family Practice”) is writing to inform you about an incident that may have involved your personal information. We take the privacy and security of our patient information very seriously. Therefore, we are writing to inform you about the incident.

What Happened?

On October 11, 2021, we discovered an attempt to interfere with our computer system. That attempt largely failed and we were able to continue serving our patient community. We immediately launched an investigation to determine what happened and what information may have been accessed by an unauthorized person during the incident. That investigation revealed, on May 21, 2022, whose information may have been involved in the incident. We have no evidence that any information has been misused.

What Information Was Involved?

The files that may have been accessed contained your name, [Redacted]. Patient medical records were not involved in the incident.

What Are We Doing?

As soon as we discovered the incident, we took the steps described above. We also reported the incident to law enforcement and will continue to cooperate with their investigation.
